Full Job Description

We are seeking a Physician Assistant or Nurse Practitioner to join the M Physician’s Memory Care team.  This is an outpatient role and will be practiced at the M Physician’s St. Louis Park location.

Practice Details:

  • Conduct comprehensive cognitive assessments, including history-taking, neurological exams, and validated screening tools.
  • Develop and adjust individualized care plans in collaboration with your Memory Care neurologists and others on the treatment team.
  • Provide education and counseling to patients and caregivers on disease progression, treatment options, and community resources.
  • Respond to urgent or emergent neurological issues related to prescribed treatments, such as anti-amyloid therapies, to evaluate and triage next steps.
  • Participate in interdisciplinary case conferences and contribute to quality improvement initiatives in memory care.
  • Ongoing collaboration with all members of our care team, patients and families

Additional Practice Details:

  • Full time 40 hours per week
  • Monday – Friday
  • No current weekend or holiday requirements

Position Qualifications:

A qualified candidate will be a graduate of an accredited school of nursing and formal Nurse Practitioner training program or a graduate of an accredited Physician Assistant program. Have a current MN RN license and be approved or eligible by the Minnesota Board of Nursing for prescription writing privileges or a current PA license with prescriptive authority through the MN Board of Medical Practice. Be certified through ANA / AANC or eligible for ANA/AANC national exam or through the NCCPA and meet threshold criteria/qualifications for credentialing and privileges.

 

Preferred Qualifications:

3 years as an advanced practice provider experience in a neurologic care setting is preferred. New graduates with an interest in neurology are encouraged to apply.
