Description

The Long Term Care Advanced Practice Provider (APP) delivers high autonomy clinical care within skilled nursing and long term care facilities, working closely with medical directors, nursing teams, and interdisciplinary partners to improve resident stability and reduce avoidable hospitalizations. Details include:
  • Perform comprehensive assessments and manage acute and chronic conditions common in LTC populations.
  • Develop and adjust individualized care plans in collaboration with facility teams.
  • Manage high risk LTC needs including infections, wounds, CHF/COPD, dementia, and polypharmacy risks.
  • Serve as a consistent clinical presence supporting workflows, new admissions, and care transitions
  • Participate in interdisciplinary care planning, education, and quality improvement initiatives.
  • Support regulatory readiness, documentation accuracy, and survey/audit preparation.
  • Utilize strong independent clinical judgment and escalate care appropriately.
