Full Job Description
This is a full time position for MLIH's Mobile Unit, with some work at physical clinic locations. This position requires regular and routine travel on the Mobile Unit.

Position Purpose

The Nurse Practitioner (NP) serves as the medical provider for the mobile health unit, providing comprehensive patient assessment, diagnosis, treatment, and follow-up care in community settings. The NP will document services in the agency's EHR, schedule patients as needed, coordinate care with staff and community partners, and support mobile unit operations. The NP will also work in the agency's physical clinic sites as assigned.

Essential Job Functions
  • Provide medical assessment, diagnosis, treatment, and follow-up within the NP scope of practice.
  • Obtain histories, perform examinations, review diagnostic results, and develop plans of treatment.
  • Prescribe medications and order tests, treatments, and referrals as needed.
  • Provide preventive care, health education, and chronic disease management.
  • Accurately document patient care in the agency's EHR.
  • Schedule patient visits and follow-up appointments in the EHR as needed.
  • Assist with patient flow and mobile unit operations.
  • Maintain a safe, clean, and organized clinical environment.
  • Participate in community outreach, screenings, and health education activities as assigned.
  • Provide medical provider services in the agency's physical clinic periodically, as assigned.
  • Perform other duties as assigned.

Minimum Qualifications
  • Current Nurse Practitioner license in West Virginia.
  • Graduate of an accredited Nurse Practitioner program.
  • Current CPR/BLS certification or ability to obtain.
  • EHR experience preferred.
  • Ability to travel to community locations as required.

Working Conditions

This position will work in the mobile health unit and/or community settings at least 80% of the time. When working at the agency, the position is performed in a climate-controlled environment. The position requires standing and moving about the agency for no more than 20% of the workday and lifting less than 50 pounds no more than 10% of the time.
