MS Nurse Practitioner or DNP Doctorate Nurse Practitioner with NYS license and national certification
Current NY State RN license required
DEA license for prescribing controlled substances in NY
Valid NY State driver's license and automobile insurance
Minimum of two years as a Nurse Practitioner, preferably in primary care or home health; hospice experience preferred
Responsibilities
Conduct in-home patient visits for symptom management and care discussions
Review clinical documents for eligibility and terminal diagnosis with medical director and intake staff
Act as the agency's on-call provider for symptom management and medication prescribing
Provide clinical expertise to nursing and other staff members
Engage in community education and outreach programs

Description
Schedule: M-F, 8:30am to 4:30pm. One (1) weekend on call per month. One (1) to two (2) holidays on-call per year.
Responsibilities may include:
Conducting in-home patient visits for symptom management, collaboration of care, goals of care discussion, assessment of complex wounds, and evaluation of eligibility for hospice benefits.
In collaboration with the medical director and intake nursing staff, reviewing clinical documents to determine eligibility and terminal diagnosis.
Serving as the agency's on-call provider, which includes advising nursing staff on symptom management, prescribing medications including controlled substances, and completing death certificates.
Serving as a clinical expert to nursing and other staff.
Participating in community education and outreach.
Requirements
Education, Skills & Experience:
MS Nurse Practitioner or DNP Doctorate Nurse Practitioner with NYS license and national certification.
Must hold a current NY State RN license.
DEA license to prescribe controlled substances in New York State.
Current State of New York driver's license and automobile insurance.
Two (2) years of Nurse Practitioner experience, preferably in primary care or home health.
Strong preference will be given to candidates with hospice experience and certification.
