Nurse Practitioner orPhysician Assistant to join our
ACE (Acute Care of Elders) teamat
IU Health West Hospital in Avon, IN! ACE is an inpatient consults program at IU Health Methodist Hospital , IU Health North Hospital , and IU Health West Hospital .
We supplement inpatient care with an emphasis on key geriatric syndromes, falls, and declines in Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
Position Details: - Full-time, Day Shift
- M-F, 8:00am-4:30pm
- No weekends or call, allowing for work/life balance
- 100% inpatient
- Expected patient volume in the hospital 10-15/week
- Team includes 1 physician and 1 advanced practice provider
- Once onboarding and inpatient practice is established, outpatient session(s) will be expected to facilitate continuum of care for patient post-hospitalization
Qualifications: - ACNP, AGNP, FNP or PA certification required
- Background of geriatrics, family medicine, hospital medicine or primary care
- Passionate about serving an aging population through a consultative model of care focused on acute, complex geriatric syndromes
