Summary: The NursePractitioner (NP) provides medically necessary primary care services andtreatments to skilled and convalescent patients in skilled, rehabilitation andlong term healthcare centers. The NP functions under the direction of acollaborating physician in accordance with state regulations. The NP providesmedical management of the residents in coordination with attending physicians,nursing and rehabilitation staff. The NPprovides clinical support and communication to residents, families, the interdisciplinaryteam and nursing center's administration.
ESSENTIALDUTIES AND RESPONSIBILITIES:- Makes visits to residents in skilled nursing facilitiesand/or assisted living facilities to provide medical care and treatment totheir residents;
- Performs physical examinations and preventive healthmeasures within prescribed guidelines and instructions of attending physiciansutilizing approved protocols;
- Orders, interprets and evaluates diagnostic tests toidentify and assess residents' clinical problems and health care needs;
- Is responsible to ensure documentation is complete andtimely for resident management/continuity of care and billing purposes.Documentation of medical necessity for purposes of billing visits;
- Is responsible to ensure communication of billinginformation to the billing service;
- Ensures that prescriptive authority and order writingare performed in accordance with applicable laws and standards;
- Develops understanding of facility programs andpolicies/procedures/guidelines;
- Communicates with interdisciplinary team and Directorof Nursing as well as medical staff to ensure continuity of care delivery;
- Monitors quality indicators, identifies areas ofopportunity and seeks solutions to enhance care and outcomes; and
- Isa resident advocate and communicates with families when appropriate.
Scope of ResponsibilityThe Nurse Practitioner:
- Is a resident advocate;
- Performs annual and routine exams as well as episodicand acute care visits when needed;
- May visit a resident during the admission process, whenthere is a medical condition which requires immediate assessment;
- Performs exams as relates to change in condition;
- Performs exams as relates to medication review andchanges;
- Performs exams pursuant to collaboration withresident's physician to manage resident's medical care;
- Provides clinical monitoring during course oftreatment;
- Completes review of physician orders for medicalnecessity, supporting diagnosis, pharmacological contraindications, generic vs.trade or OTC;
- Participates in psychotropic medical management andbehavior management process;
- Maintainsprescriptive authority;
- Providesclinical monitoring;
- Providesongoing clinical management;
- Ensuresdocumentation is complete, and meets medical necessity and billing guidelines;
- Participatesin the survey process;
- Understandsfacility clinical systems and processes;
- CompletesMedicare certifications as necessary;
- Mustmaintain active licensure status and current continuing education credits as requiredby law;
- Performsother duties as assigned including education and quality management support;and
- Mayprovide services such as employee physicals and in-services pursuant tofacility agreement.
Compliance Responsibilities The Nurse Practitioner:
- Complies with applicable legal requirements, standards,policies and procedures including but not limited those within the CorporateCompliance Program, Corporate Code of Conduct, HIPAA, and Federal False ClaimsAct.
- Attends the center's orientation and training programsas required;
- 3. Reports concerns and suspected incidences ofnon-compliance immediately to theCorporate Chief Compliance Officer.
- Cooperates with monitoring and audit functions andinvestigations; and
- Participates, as requested, in process improvementresponsibilities.

Professional Experience/Educational Requirements- Registered nurse who is mastered prepared as anadvanced practitioner
- A minimum 5 years clinical experience as a registerednurse and 3 years as an APRN is preferred.
Geriatric nursing experience is a plus
Certification/Licensure- Registered nurse who is mastered prepared andcurrently licensed as an advanced practitioner in the state in which practicewill occur.
- Current accreditation from a nationalcertification organization
- Eligible for prescriptive privileges
- Active NPI number
- If registered nurse and "grandfathered", allother credentialing requirements in the state in which practice willoccur.
