Full Job Description
We are seeking a compassionate and skilled Primary Care Nurse Practitioner to join our medical group on a full time basis. The ideal candidate will provide comprehensive primary care services to our senior residents, ensuring they receive the highest quality of medical care in our sub-acute, long term care, assisted living, and medical office settings. This role involves collaborating with the physician other healthcare professionals to develop and implement individualized care plans, promoting wellness, and managing chronic conditions. The candidate will be expected to work three days at Applewood Village Freehold NJ and one day at Pines Village Whiting NJ. We offer a very supportive work environment that includes a team of well-qualified medical, nursing, and multi disciplinary professionals.

Essential Functions:
  • Conduct thorough assessments and physical examinations of senior residents.
  • Diagnose and manage acute and chronic health conditions.
  • Develop, implement, and monitor individualized care plans.
  • Prescribe medications and treatments as necessary.
  • Provide health education and counseling to residents and their families.
  • Collaborate with physicians, nurses, and other healthcare team members.
  • Maintain accurate and up-to-date medical records.
  • Participate in care team meetings and contribute to quality improvement initiatives.
  • Ensure compliance with all regulatory and organizational policies and procedures.
