Job Description

Purpose of Position:

Under general supervision, serves as a licensed, independent practitioner responsible for planning, coordinating, delivering, and managing comprehensive health care services in a health care setting. Ensures patient and family-centered quality care to improve outcomes and patient experience, in accordance with applicable NYC Health + Hospitals policies and procedures, state and federal rules and regulations, and nursing standards of care.

Examples of Typical Tasks:
1. Provides continuous comprehensive medical and nursing care, including preventive and supportive health care services to patients and families.
2. Manages health problems and coordinates health care for various patient populations. Identifies and diagnoses illnesses and physical conditions, and implements therapeutic and corrective measures.
3. Consistent with the established nursing practice, differentiate between normal and abnormal findings in the screening process to detect changes in health status that require physician's intervention.
4. Develops and maintains partnerships with community-based organizations and makes referrals to consulting physicians and/or other health care professionals and community agencies, as necessary.
5. Evaluate and interpret patient data, in order to plan and implement appropriate nursing interventions.
6. Consult with physicians and other health care practitioners in the evaluation of health screening findings to make prospective decisions about treatment that will best support improved outcomes and patient satisfaction.
7. Formulates differential diagnosis and the plan of care based on medical history, physical exams, nursing assessment, and other pertinent information. Identifies health and medical risks and needs.
8. Develop, implement, and evaluate patient plan of care. Determines effectiveness and modifies as per clinical standards and practice protocols, where needed, to achieve medical and health goals.
9. Orders, conducts and interprets diagnostic and laboratory tests. Identifies and may prescribe appropriate pharmacological and non-pharmacological interventions and therapies.
10. Manages a patient session schedule allowing for both scheduled and non-emergent unscheduled visits; maintains a work schedule that ensures availability and continuity of care; ensures optimal availability and utilization of appropriate resources.
11. Provide education, guidance, and direction to nursing staff and other members of the health care team to ensure quality and safe medical and nursing care is provided.
12. Provides education and counseling to patients and families regarding health status, treatment and progress, medication management, disease prevention, and health promotion, records, and maintains a system of relevant documentation.
13. Performs other related duties, as directed.

Endocrinology Department:

We are seeking a dedicated and knowledgeable Nurse Practitioner to join our Endocrinology team. As a Nurse Practitioner in this role, you will collaborate with our multidisciplinary team of endocrinologists, nurses, and other healthcare professionals to provide comprehensive care to patients with endocrine disorders. You will play a critical role in evaluating, diagnosing, and managing hormonal and metabolic conditions, promoting patient education and self-management, and optimizing health outcomes for individuals across the lifespan.

Essential Duties:

Conduct thorough assessments of patients presenting with endocrine-related concerns, including medical history reviews, physical examinations, and interpretation of diagnostic tests and laboratory results.

Collaborate with endocrinologists and other healthcare providers to formulate individualized treatment plans for patients with endocrine disorders, incorporating evidence-based guidelines and best practices.

Provide education and counseling to patients and their families on the nature of their endocrine condition, treatment options, medication management, lifestyle modifications, and self-care strategies.

Perform diagnostic and therapeutic procedures as indicated, including but not limited to hormone level monitoring, insulin administration, thyroid ultrasound-guided biopsies, and interpretation of bone density scans.

Monitor patients' response to treatment and disease progression over time, adjusting management plans as needed to achieve optimal clinical outcomes and improve quality of life.

Coordinate care across healthcare settings and specialties, facilitating referrals to other healthcare providers or ancillary services as appropriate to address patients' holistic needs.

Participate in multidisciplinary team meetings, case conferences, and quality improvement initiatives to enhance the delivery of patient-centered care and promote continuous improvement in clinical practice.

Maintain accurate and up-to-date medical records, documenting assessments, interventions, and patient communications in accordance with regulatory and organizational standards.

Stay abreast of advances in endocrinology research, pharmacotherapy, and treatment modalities through ongoing professional development activities, such as attending conferences, seminars, and continuing education programs.

Minimum Qualifications
1. A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
2. Current certificate as a Nurse Practitioner issued by the NYSED; and
3. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

Additional Qualifications

Additional Preferences:

Strongly prefer National Board Certification in Family Health Nurse Practitioner (NP) Certification by the American Nurses Credentialing Center (ANCCB) or American Academy of Nurse Practitioners Certification Board (AANPCB).

Departmental Preferences:

Two clinical years experience as Family Health Nurse Practitioner (NP) preferred.

An annual education differential if you currently possess an appropriate degree in nursing or an allied health discipline from an accredited college or university, as listed below:
  • Bachelors of Science in Nursing (BSN) or Bachelors in allied health discipline = $900
  • Masters of Science in Nursing (MSN) or Masters in allied health discipline = $1,200
  • Doctorate in Education, Nursing Practice or PhD in Nursing (DNP) = $2,500

An annual certification differential of $1,878 if you currently hold an appropriate certification that is nationally recognized in the discipline or area of specialization for which you are employed.

An annual nursing experience differential ranging from $1,097 up to $9,100 for up to 10 years of experience if you have outside nursing experience as a Nurse Practitioner.

For NP (PC/BH) ONLY: An annual assignment differential ranging from $10,000 up to $25,000 if assigned to a Behavioral Health department AND regularly working evenings/nights
