Job Description

We are seeking a dedicated and skilled Family Nurse Practitioner or Physician Assistant to join our healthcare team. The position includes a 4 day work week with no holidays or weekends. The ideal candidate will possess a comprehensive understanding of patient care with experience across various medical settings. This role involves providing high-quality healthcare services to patients of all ages, conducting assessments and implementing treatment plans tailored to individual needs. The Family Nurse Practitioner or Physicians Assistant will work collaboratively with other healthcare professionals to ensure optimal patient outcomes.

Duties:
  • Conduct thorough physical examinations and patient assessments.
  • Develop and implement individualized care plans based on clinical evaluations.
  • Administer medications, including injections and IV infusions as necessary.
  • Perform diagnostic evaluations and interpret results for effective treatment planning.
  • Provide acute care management, including triage and emergency interventions.
  • Manage chronic conditions and offer health coaching to promote wellness.
  • Document patient interactions accurately in electronic health records (EHR) systems.
  • Collaborate with multidisciplinary teams in various settings such as outpatient clinics, hospitals and assisted living facilities.


Experience:

Candidates should have relevant experience in the following areas:
  • Medical office experience with familiarity in EMR systems such as Epic.
  • Experience in pediatrics, geriatrics or family planning is highly desirable.
  • Knowledge of medical terminology, ICD coding (ICD-9/ICD-10) CPT coding and HIPAA regulations.
  • Familiarity with specialties including gastroenterology, infectious disease care, pain management, behavioral health and hospice & palliative medicine is a plus.
  • Experience working with diverse populations including those with developmental disabilities or chronic illnesses.
