Job OverviewThe Nurse Practitioner, Care Management, provides care management and primary care medical services to high-risk patients. The NP manages the assigned patient panel.
Minimum Requirements- Nurse practitioner with an active, unrestricted CA license to practice medicine by the Nursing Board of California.
- Minimum 2 years of experience and training as a nurse practitioner.
- Minimum of 1 year experience in a high-risk specialty, Geriatric, Hospice, and/or Palliative Medicine, preferred.
- Requires a valid and active license to practice medicine in the State of California.
- Previous experience with computer-based systems required; Electronic Medical Records experience preferred.
- A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.
Compensation$138,257.60 - $172,806.40 annually
