Providence Medical Foundation is seeking a dedicated nurse practitioner to join its growing Anticoagulation Clinic team that will serve patients across California. The base location can be at Anaheim or Mission Hills, California. The Anticoagulation Nurse Practitioner (NP) provides comprehensive, evidence-based management of patients receiving anticoagulant therapy in the ambulatory setting. The NP independently evaluates patients, develops and implements anticoagulation treatment plans, orders and interprets laboratory testing, adjusts anticoagulant therapy, prescribes medications within authorized scope of practice, and manages peri-procedural anticoagulation.
This hybrid role offers the opportunity to provide specialized anticoagulation management through a telehealth-focused practice while helping build and shape a new program. The ideal candidate will be adaptable, comfortable working in a developing environment and excited to contribute to best practices across the organization.
Essential Functions:
- Provide comprehensive management of patients receiving anticoagulation therapy, including warfarin and DOACs
- Assess patients, order and interpret appropriate testing, prescribe and adjust therapy, and manage anticoagulation-related clinical concerns
- Manage peri-procedural anticoagulation and coordinate care across specialties and transitions of care
- Collaborate with physicians, pharmacists, nurses, and other healthcare professionals to provide safe, coordinated anticoagulation care
- Partner with program leadership to help develop, implement, and strengthen a new anticoagulation model, including clinical protocols, workflows, referral pathways, and standards of care
- Identify opportunities to expand and improve the program through quality improvement, patient outreach, care coordination, and outcome monitoring
- Serve as a clinical resource and champion for anticoagulation best practices across the organization
Position Details:
- Full-time outpatient position
- Patient interactions are fully through telehealth
- Monday to Friday schedule; 4 days in business office and 1 day remote
Compensation & Benefits:
- Compensation is between $170,080 and $224,215 per year
The posted salary reflects the starting range of total compensation and may include productivity bonuses, incentives related to quality and performance, hospital/ED call stipends, extra shift incentives, and other forms of cash compensation as applicable to the position. In addition, providers typically receive a CME allowance and other benefits offered by their medical group employer. Please note that this salary range is provided in accordance with State law and is subject to variation due to the factors noted above.
