JOB DESCRIPTION

Nurse Navigator (RN) General Surgeries
Job Type: FTE | Days
Wage scale: $37.93 – $70.30 per hour

How you’ll contribute 
As a Nurse Navigator (RN), you will serve as a patient's guide through the healthcare system, helping coordinate care, remove barriers, and improve the overall patient experience. 

Job Duties

  • Serve as the primary point of contact for patients and families
  • Coordinate appointments, testing, treatments, and referrals
  • Provide education about diagnoses, treatment options, and available resources
  • Assess barriers to care and connect patients with community resources and support services
  • Advocate for patients and helping ensure timely, appropriate care across the continuum
  • Provide hands-on patient care including but not limited to vitals, patient intake, in-clinic procedures
  • Cover other clinics as necessary
  • Other duties as assigned
