Description of Job
The Work Unit:

Within DCPHR's Health Equity Branch, the Mobile Public Health Clinic Program (MPHCP) is responsible for bridging gaps in access to disease control and public health services statewide, and rapidly responding to emergent issues. The program provides services to all Coloradans with an emphasis on serving Black, Indigenous, and People of Color (BIPOC), low income people, uninsured people and underserved communities such as those in rural areas of the state or with high social vulnerability. On a day-to-day basis, vaccine clinics for routine, COVID-19, flu and mpox vaccines are our core service offering. Other disease control services that can be provided include provision of post-exposure prophylaxis (e.g. immune globulin (IG) to people who can't be vaccinated), treatments and testing services during outbreaks or disease exposure incidents. During times of emergency, MPHC can quickly deploy temporary clinics in places that are most convenient and accessible for the population affected.
What You Will Do:

This position coordinates and leads patient care services at clinics. Guiding nurses and clinical staff, providing ongoing training to help ensure the team maintains best practices. Traveling to different regions of the State of Colorado, serving underserved populations, bringing public health resources and disease mitigation through vaccinations, testing, and other disease control services. Requiring high levels of engagement with underserved communities, direct patient care for these populations, and managing personal health information.
Key Responsibilities:

• Supervises and coordinates patient care services before and at assigned clinics.
• Provides direct patient care to the public according to MPHCP protocols, standing orders and required training.
• Guides and support other nurses and clinical staff, provide ongoing training, guidance and helps to ensure the team maintains best practices in patient care, documentation, and use of clinical equipment.
• Immunizations, immune globulin (IG) administration during outbreaks for post-exposure protection to people who aren't appropriately vaccinated or can't be vaccinated (e.g. hepatitis A and measles), and testing services (e.g. respiratory viruses, mpox, food and waterborne illnesses, tuberculosis, vaccine preventable diseases, etc.) during outbreaks of disease exposure incidents. Clinical Screenings as guided by orders.
• Provides oversight for all inventory, vaccine and equipment within the mobile vans including and as necessary, clinic set-up and take-down duties.
• Oversees all aspects of patient privacy including communications, considerations for CDPHE's physical presence within the community, and the proper collection and protection of patient information, ensuring that HIPAA privacy rules are enforced.
• Ensures standing orders and protocols are followed including medical management plans for vaccine reactions.
• Reviews CIIS and patient-provided records to assess vaccines needed at each visit and screens for contraindications to vaccines.
• Provides immunization education and vaccine information statements (VIS) to a patient, parent, or legal representative prior to vaccine administration.
• Responsible for reporting clinically significant adverse events and vaccine administration errors through the Federal Vaccine Adverse Event Reporting System (VAERS).
• Provides patient centered care in a way that maintains cultural humility through effective communication and interaction with people across diverse cultures and through patients' preferred language.
• Facilitates patient decision making and follows nursing protocols and standing orders under the direction of a medical director.
• Ensures testing protocols are followed with regards to: collecting all required patient data, test ordering, and specimen collection, storage, and transportation.
• Drives a cargo sized mobile public health clinic van to and from the vaccine sites, sometimes over multiple days and sometimes through reasonable winter weather conditions.
• Collects testing specimens, including but not limited to nasal, oral, or other site swabs, sputum collection, stool collection, venipuncture for blood testing, or other specified collection for outbreak response. complete and timely data entry into appropriate systems (Patagonia (or current EHR), CIIS, EpiTrax, LabOnline) Provide specific examination.
• This position is responsible for the stewardship, dispensing and dose-level accountability for 317 vaccines, Vaccines for Children (VFC) vaccines, privately funded vaccines for insured patients, and other publicly and state-funded vaccines offered through the MPHCP.
• Maintains and monitors vaccine storage equipment during transport and on-site at the clinic.
• Reads and records vaccine storage unit minimum/maximum temperatures at recommended intervals.
• Follows all off-site clinic requirements and guidance from the VFC program.
• Follows the Routine Vaccine Management Plan and the Emergency Vaccine Management Plan.
• Is responsible for proper VFC, 317, and state-funded eligibility screening and documentation procedures.
• Maintains the organization of vaccines within storage units, keeping VFC, 317, and state-funded vaccines labeled and separate from privately purchased vaccines.
• Takes immediate action to respond when vaccines are exposed to out-of-range temperatures; documents any temperature excursion and all actions taken, notifies the VFC Program, and supplies all follow up documentation in a timely manner.
• Responsible for screening each patient for VFC, 317 or state vaccine eligibility.
• Identify temperature excursions, report temperature excursions, and work with the VFC program to resolve temperature excursions.
• Maintains cultural humility while serving communities around the state.
• Updates vaccine administration system, Patagonia, EHR, CIIS, and internal trackers with administrative data from clinics in a timely manner.
• Provides reports after each clinic with metrics including doses given, attendance, etc.
• Stocks the mobile clinic vans with needed supplies, and ensures vans are maintained for safe driving by reporting any concerns, maintenance or repair needs to the Program Manager.
• Maintains a travel card to manage travel expenses, completes relevant reimbursement forms, as required (if a travel card is requested).

Education/Licensure/Certification/Experience:
Master's degree from an accredited institution in a epidemiology or nursing, public health, nurse administrator, nurse researcher and Current, valid licensure as a registered nurse from the Colorado Board of Nursing or any state participating in the Enhanced Nurse Licensure Compact (eNLC) and Two (2) years of professional experience in communicable disease , public health, infection prevention control or in an occupational field related to the work assigned to the position.

Substitutions
Additional appropriate experience in communicable disease, public health, infection prevention control will substitute for the degree requirement on a year-for-year basis
