POSITION PURPOSE

TheVascular SurgeryNP/PA Surgeryprovides health care services under the direction ofcollaboratingsurgeonsin the Department of Surgeryunder the direction oftheAdvanced Practice Professional team (APP).Under the direction of the surgeonand the Director of APPs, the NP/PAisresponsibleforcoordinatingthehealth care plan for the assigned patient population by collaborating with consulting physicians and health care professionals.The NP/PAcarries out physician orders andmaintains the continuum of care through the coordination and integration of all phases of patient care, guided byprogress notes,consults,care plans, critical pathways, managed care guidelines, best practice protocols, evidenced based care, and the utilization review processes.In partnership with the supervisingphysician,theNP/PAevaluates care based on quality and core measures,accessand cost effectiveness.TheNP/PAcontributes to excellence in patient care by focusing on age specific,developmentaland spiritual needs and serving as a role model in patient care, consulting, teaching, and leadership.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

Clinical care forVascular Surgerypatients:

• Formulate and implement medical plans of care for acute management of all Surgery (inpatient, emergency department, and consult) patients with collaborating physicians.
• Assistwith evaluation and triage of emergency room patients.
• Provide direct clinical services forsurgicalpatients according to established protocols, using evidence-based practice and clinical practice standards, under the supervision of the collaborating physicians.
• Patient work-up, including history and physical examination with ordering ofappropriate laboratorytests, x-ray studies and other diagnostic studies asindicated.
• Completion of daily hospital work rounds includingpre and post operationsand charting of progress notes asrequiredor as assigned.
• Completion of discharge summaries, chart rounds, staffing conference summaries and consults as directed by service needs.
• Participation in the care of off-service patients asindicatedby supervising physician request.
• Orderappropriate diagnosticand laboratory dataunder the direction of the supervising physicianand communicate results to physicians and other health care providers.
• Assess changes in patient status and institutes routine and emergency interventions to stabilize patientsunder the direction of the supervising physician and APP team.
• Document plan of care and patient progress in medical records and narrative summaries.
• Evaluate and document patient outcomes and revises the plan under the direction of collaborating physicians in association with other specialty consultants.
• Help facilitatedischarge planning along with patients, families, other members of the health care team and referral agencies.

Collaborative role with other disciplines:

• Act as the daily central liaison to all health care team members for patient care concerns
• Coordinate speech, physical and occupational therapies
• Participate in patient care conferences and peer quality review with collaborating physician, consulting medical teams, and other health care disciplines

Education Focus:

• Explain the purpose,preparationand potential effects of the procedure to patient and families
• Provide pre-operative teaching and discharge planning to patients and families.
• Design, instruct, andreviewinformation and programs pertinent totheinpatient and outpatientsurgicalpopulation.
• Participateonhospital,nursingand medical committees as designated.
• Serve as a hospital resource to staff, patients, and families.
• Develop and implement a personal continuing education program to maintain current knowledge of surgical care and associated medical concerns.
• Maintain good rapport and cooperative relationships.
• Approachconflict in a constructive manner.
• Help toidentifyproblems, offer solutions, andparticipatein theirresolution.
• Maintain the confidentiality of informationacquiredpertaining topatients, physicians,employeesand visitors toTrinity Health Ann Arbor.
• Discuss patient and hospital information only amongappropriate personnel appropriately private places.
• Followall HIPAA policies and procedures.

REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE

NPEducation:

Masters Degree in clinical nursingfrom an accreditedAcute CareNurse Practitioner Program with emphasis on area of certification/licensure.

NPLicensure:

Current certification by a nationally accredited professional nursing organization such as American Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP). Successfully pass the certification exam fromaccreditedcertifyingorganization approved by the Michigan Board of Nursingrelativeto the CNP's designated specialty. Acceptable certifications: Acute Care Adult Gerontology Acute Care and Adult NP. Alternate certifications prior to January 2019 may be accepted or approved if currently practicing at a Trinity Health facility or if there is adequate clinical experience insubspecialtyarea.

PA Education:

Requiresgraduation from an ARC-PA accredited PA program.

PA Licensure:

Current License/Certification asPhysicianAssistant with the State of Michigan.

Experience:

Surgical experiencepreferred, butweare willing to train the right candidate.
