GENERAL PURPOSE:

Provides individualized assistance, support and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as first point of contact for patients and families.

The Nurse Navigator will work collaboratively with the NICU interdisciplinary team, il equipment, and other specialized care, by coordinating teaching schedncluding Nurse Navigators, bedside nurses, medical providers, and other support services, to coordinate and provide patient and family education throughout the NICU stay and in preparation for discharge or transfer. The role focuses on families with complex discharge needs, including home oxygen, feeding tubes, medications, medicaules, providing direct caregiver education, and supporting accurate documentation of education and caregiver readiness. The Patient & Family Education Navigator also supports continuity of care for patients transferring to other inpatient units and may assist with Nurse Navigator functions as needed, including coordination of appointments, referrals, home care services, and medical equipment vendors. Through collaboration with the NICU Family Advisor, developmental care specialists, and other members of the healthcare team, this role helps identify barriers to discharge, strengthen family preparedness, and enhance the overall patient and family experience. A minimum of 3 years of NICU nursing experience is required. BSN required. Active NYS RN license required. All interested applicants submit resume to Nursing recruitment, Box 619-19.
