Your Role on the TeamPosition Summary: The Medication Management Pharmacist is responsible for the provision of patient-centered clinical services, including review of patient medical records, performance of comprehensive medication therapy review, provision of medication information/education to providers and patients, application of evidenced-based medicine and national guidelines to optimize medication-related health outcomes. The Medication Management Pharmacist will provide care for patients in-person and virtually via video, telephone, or text as part of a pharmacist-led, referral-based, Medication Management Clinic. The role will assure the appropriate selection, dosing, administration, and dispensing of medication orders and work under a collaborative practice agreement to autonomously manage referred patients. The MM Pharmacist will serve as a subject matter expert and complete comprehensive medication reviews for patients with specific high-risk medication concerns and high-risk disease states and patients who are at high risk for rehospitalization. This position is a Direct Care Provider as defined in Pharmacy Policy. The MM Pharmacist may perform prior authorization reviews of high-cost medications and deferred authorization requests, as needed. The MM Pharmacist is responsible for supervising pharmacy technicians, ensuring completion of medication histories, and actively precepting pharmacy residents, students, technicians, or interns as assigned by the supervisor. What It Takes to be SuccessfulRequired Qualifications:Registered Pharmacist in good standing licensed by the state of California Current BLS or ACLS upon hire Completion of a Doctor of Pharmacy degree from an accredited School of Pharmacy. Completion of ASHP-accredited PGY1 pharmacy practice residency and a PGY2 Ambulatory Care Pharmacy Residency or 2+ years experience as a clinical pharmacist in an ambulatory setting Preferred Qualifications:Training and knowledge of specialty areas (oncology, hepatology/gastroenterology, neurology, dermatology) Knowledge of 340B program Four or more years of pharmacy experience. Specialty/retail pharmacy, ambulatory clinic pharmacy, and/or PBM experience.
