MDS Coordinator (LPN/RN)

Carlin Springs Health & Rehabilitation is seeking an experienced MDS Coordinator (LPN or RN) to join our dedicated team! If you are detail-oriented, organized, and passionate about providing exceptional care to residents in a skilled nursing environment, we'd love to hear from you.
Pay

LPN: $44.00 - $49.00 per hour
RN: $101,000 - $110,000 annually
Position Summary

The MDS Coordinator is responsible for coordinating and completing the Resident Assessment Instrument (RAI)/MDS process to ensure accurate, timely, and compliant assessments. This role works closely with the interdisciplinary team to support quality resident care, reimbursement, and regulatory compliance.
Qualifications

  • Minimum 1 year of MDS experience required
  • Experience in a skilled nursing or long-term care setting preferred
