Full Job Description
Alliance Health at Baldwinville is hiring an experienced MDS Coordinator to manage resident assessments and care planning at our quaint, 95 bed, skilled nursing/long-term care facility. As the backbone to our multidisciplinary care approach, you have an opportunity to make a real difference in our Residents' lives. Due to our continued growth, we are looking for team-oriented individuals to join our clinical team.

The MDS Coordinator Nurse will be responsible for, but not limited to:
  • Completion of the OBRA, MDS and CAA's per schedule
  • Participate in the development and ensured completion of the Care Plan, identifying the individual needs of the residents.
  • Facilitate weekly managed care meetings, participate in weekly care plan meetings, and other meetings as assigned.
  • Assist with monitoring of resident documentation.
  • Assist in ensuring C.N.A documentation is reflective of resident care.
  • Ensure accurate coding and supporting documentation
  • Maintain CMS and MA compliance
  • Support audits and survey readiness

Qualifications:
  • Must have active nursing license in MA and one to two years of MDS experience working in an LTC or Sub-acute setting preferred.
  • MDS experience preferred
  • Long-term care experience
  • Strong documentation and organizational skills

Compensation and Benefits
  • $90,000 - $102,000 per year (based on experience)
