Manager, Value-Based Care

Premier Community HealthCare Group is seeking a strategic, mission-driven Manager, Value-Based Care (VBC) to lead value-based performance across our managed-care and Accountable Care Organization (ACO) portfolios. This leadership role connects clinical operations, risk coding, data analytics, revenue cycle, and senior leadership to improve outcomes and strengthen contract performance. You will own key VBC metrics, turn data into action, close care gaps, and help Premier reach and exceed contract targets. As a member of the leadership team, you will serve as Premier's VBC subject-matter expert and help advance value-based care across the organization.

We are seeking a Manager, Value-Based Care who wants to:
• Lead a growing value-based care strategy that improves both patient outcomes and organizational performance.
• Build strong partnerships across clinical, operational, quality, coding, analytics, and revenue cycle teams.
• Turn performance data and identified gaps into measurable action and results.
• Help advance accessible, equitable healthcare for the communities Premier serves.

What You'll Do
• Lead Premier's VBC program and partner with Senior Leadership on strategy, growth, and sustainability.
• Drive provider engagement, risk-score capture, coding accuracy, and resolution of documentation and continuity-of-care gaps.
• Own VBC dashboards, payer scorecards, ACO data, risk scores, seen rates, and care-gap performance.
• Lead cross-functional gap-closure and coding huddles and prioritize the highest-value opportunities.
• Oversee AWV, newly attributed-member, patient outreach, and redetermination-retention workflows.
• Partner with Quality and Marketing to develop campaigns that improve care-gap closure and patient-seen rates.
• Optimize Epic alerts, registries, and supplemental-data workflows so delivered care is accurately captured and credited.

Qualifications
• Bachelor's degree in Health Information Management, Public Health, Healthcare Administration, Nursing, or related field; or 10+ years of relevant experience as determined by Human Resources.
• High School diploma with commitment to obtain a degree within a reasonable period may be considered under PCHG guidelines.
• Five years of healthcare experience, including two years in value-based care, population health, quality improvement, or risk adjustment.
• Experience leading teams or projects.
• Strong understanding of value-based care, population health, quality measures, care gaps, and performance metrics.
• Ability to analyze data, identify opportunities, and develop actionable strategies.

Bonus Skills
• FQHC, community health, or ACO experience.
• CRC, CPC, or CDIP certification.
• Epic experience and ability to maintain Epic certification and annual compliance.

Working Conditions & Physical Requirements
• Ability to lift 20 lbs. regularly and 30-50 lbs. occasionally.
• Ability to sit for long periods of time.
• Direct exposure to computer screens.
• May be exposed to contagious/infectious diseases.
• Ability to travel between PCHG sites for meetings, as necessary.
