Responsibilities

  • Receive and assess physician referrals for appropriateness to present to the physician team.
  • Perform clinical and socio-economic assessments on referred patients.
  • Navigate patient care between physicians and diagnostic services.
  • Connect patients and families with educational and clinical resources throughout treatment.
  • Manage Valve Clinic operations including health record management and patient-physician relationships.
  • Conduct effective follow-up with post-TAVR patients and enter findings into the TVT Registry.
  • Coordinate with marketing and CME coordinators to enhance community education and referrals.

Full Job Description
Receives physician referrals into the program, compiles clinical data, and assesses appropriateness for presentation to the multi-specialty physician team. Performs clinical and socio-economic assessments on referred patients and navigates care between physicians and diagnostic services. Connects patients and families with resources for education, support, and clinical needs throughout their course of treatment. Works collaboratively with Patient Accounting and Coding to ensure compliance with patient charges and appropriate coding of procedures. Manages Valve Clinic operations, including referral algorithms, health record management, and patient-physician relationships. Provides effective follow-up with post-TAVR patients, including 1-month and 1-year follow-up appointments, and records findings into the TVT Registry as appropriate. Coaches healthcare providers on necessary components of TVT documentation. Coordinates and hosts weekly multi-specialty physician team meetings, formulates agendas, and follows up on recommendations to facilitate patient care. Engages with Marketing and CME Coordinator to develop programs that enhance referrals and provide community education. Ensures continued compliance and certification renewal for ACC Transcatheter Valve Accreditation requirements. Collects and analyzes key registry and clinical data to improve clinical and strategic outcomes. Performs other duties as assigned. Knowledge, Skills, and Abilities: • Skilled in critical care nursing and operating room or catheterization lab experience [Required] • Basic cardiovascular rhythm recognition and cardiopulmonary monitoring assessment [Required] • Medication management with acute-care cardiovascular drugs [Required] • Advanced knowledge of cardiovascular diagnostics and imaging techniques [Required] • Excellent interpersonal and communication skills, written and verbal; [Required] • Highly organized in compiling clinical data/images for presentation to multi-specialty physician teams to accurately assess patients for advanced clinical procedures; [Preferred] • Advanced knowledge of indicators and criteria for data abstraction into the Society of Thoracic Surgeons and American College of Cardiology - National Cardiovascular Data Registry TVT Registry [Preferred] • Good computer skills [Required] • Demonstrated skill in the use of written and oral communication [Required] • Ability to write objectives, content outlines, and bibliographies desirable.
