Summary
Manages daily operations of the program, manages program compliance with applicable regulatory agencies, utilizes resources to plan and predict rehabilitation patient outcomes and quality indicators, supports team work and interdisciplinary care, coordinates and leads programmatic meetings, reviews and shares outcomes with the team.

Job Duties
  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.(Do not delete, move or over-write this statement)
  • Responsible for local efforts to achieve desired outcomes, in the building and alongside staff.
  • Implements programmatic operational and business initiatives per program director.
  • Participates in Daily Admissions call. Participates in Daily Safety Huddle.
  • Manages daily operations of the program: Supports team in facilitating DCs, lightening rounds, team conferences, family meetings where needed.
  • Under direction of the Program Director - Responsible for the local outcomes such as DC to community, Safety, Patient Experience: Presents at ROQUE/supports other team members understanding of data and presentation at ROQUEe.
  • Utilization Review-manages appropriate LOS while maintaining high quality care for patients in their program(s).
  • Continuously monitors program compliance for TJC, CARF, CMS, and other applicable regulations.
  • Tours families/caregivers of potential patients.
  • Provides onsite support to Program Staff. Supports team relationships, teamwork, interdisciplinary care.
  • Responsibility for hiring and support of key positions may be required.
  • Plans and oversees community activities in collaboration with Program Director: advocacy groups, education, marketing.
  • Other duties as assigned-An example is Enterprise falls committee and Enterprise initiatives pertaining to program areas.
  • Provides Support to Program Director as Needed
  • Establishes mutually beneficial relationships with their program counterparts at other sites.
  • Coordinates and leads onsite programmatic meetings along with Program Director and assures structure in place for systematic communication with program team members.

Minimum Qualifications
  • Bachelor’s Degree
  • 5 years in leadership position in post-acute sector overseeing patient care programs. Working knowledge of EMR with a minimum of 5+ years in area of specialty

Preferred Qualifications
  • Master’s Degree MBA/MHA Preferred

Additional Qualifications
  • Proficient in Microsoft office, Word, PowerPoint, Excel

Physical Demands
Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
