Full Job Description
Summary:
  • The Quality and Patient Safety Manager is responsible for establishing and implementing quality, patient safety, infection control and regulatory structures and processes that support high quality and safe patient care across Penn Medicine Home Health. They provide leadership related to quality measurement, performance improvement, and regulatory compliance for the related agency. This individual is accountable for the collection, analysis, interpretation, and dissemination of quality and compliance data findings; contributes to the evaluation of quality trends, and provides leadership, management, and facilitation of quality improvement teams. This role is responsible for establishing and supporting systems designed to ensure that patient care is consistent with standards of care and regulatory requirements, assuring that the organization advances the culture, processes and standards to achieve clinical excellence and designing and implementing mechanisms that accurately measure patient outcomes which may include, patient satisfaction, clinical and functional outcomes, and infection control.

Responsibilities:
  • Facilitate improvement in patient satisfaction, patient safety, clinical quality and operations.
  • Collaborate with leadership team to develop, write and approve annual QAPI plan Perform infection control monitoring and develop, write and approve annual Infection Control Plan.
  • Assists in survey readiness activities related to quality including chart review, standards interpretation, improvement activities and monitoring activities Support safety which may include - event review, event investigation, RCA/ACA.
  • Promotes a culture of safety: assists with the Patient Safety Culture Survey and action plan
  • Serve as a change-agent to programs in their efforts for quality/safety improvement Maintain quality dashboards through the collection, collation and reporting of quality measures including patient satisfaction, standard quality metrics, falls, infection control and safety events Understand, disseminate and communicate trends in data.

Credentials:

  • Registered nurse - Required
  • Six Sigma Green Belt/Lean - Preferred

Education or Equivalent Experience:

  • Bachelor of Arts or Science (Required)
  • 3+ years Progressive healthcare experience requiring project management, data analysis, communication, and improvement strategies.
  • Knowledge and experience in quality improvement modes, such as Lean/Six Sigma, the IHI Model for Improvement, or Human Factors Engineering is strongly preferred
  • Participation in at least one regulatory survey such as Joint Commission, CMS, or Pennsylvania Department of Health is required.
  • Knowledge of Joint Commission, CMS, and PA DOH standard/regulations. Knowledge of national standards and outcomes for quality, safety and healthcare related to discipline of care (ie.Home/Hospice).
  • Experience with Root Cause Analysis.
