Job Summary and Qualifications- As Manager for Case Management, you will report to the Case Management Director and will be responsible for promoting patient-centered care by coordinating all aspects of hospital-based case management within your assigned area of responsibility in alignment with the goals of the Case Management Department
- You will provide day-to-day oversight and management of the case management program, ensuring effective coordination, supervision, and administrative support of the case management team
- Acting as a case management subject-matter expert, you will serve as a resource, advisor and leader for the members of the case management team
- You will supervise and monitor professional and support staff, and ensure effective care coordination and case management practices consistent with hospital policies, applicable regulations and guidelines
- You will evaluate the quality of interactions, documentation and assessments conducted by the case management staff and facilitate initiatives that promote quality outcomes and improve patient throughput while supporting a balance of optimal care and appropriate resource utilization
What qualifications you will need:
- Bachelors in Nursing OR Masters in Social Work required
- 5 years of case management in an acute hospital setting required
- Thorough knowledge of Case Management defined by established rules and regulations of federal and non-federal programs, and understanding of hospital reimbursement as it relates to different payers such as Medicare, Medi-cal, Managed Care and private insurance plans
