Minimum Hourly Rate: $ 46.74

SUMMARY OF POSITION:

The Manager Home Recovery Care Program will be responsible for oversight of the care model that will allow patients who are clinically appropriate to have the option to receive acute care in the comfort of their own homes in lieu of an inpatient setting. This role will work directly with patient and the clinical teams on the floor of the hospital and virtually from an office setting. Home Recovery works to improve the health and well-being of patients and ease the burden on caretakers. This role will be responsible for the direction of clinical services as well as the financial management for High Acuity Care Virtual Health services. This role is accountable for measuring, monitoring and achieving KPIs and clinical performance. This role provides supervision to the clinical staff who ensure the delivery of quality care to patients. Responsible for analyzing, recommending and implementing clinical and operational improvement opportunities for the high acuity care program.

MINIMUM QUALIFICATION(S):

  • Associate's Degree in Nursing
  • Five (5) years of adult acute inpatient medical or medical surgical care experience.
  • Five (5) years of leadership experience required
  • Currently licensed to practice as a registered nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact
