Full Job Description
We are looking for a leader who is passionate about health and well-being for the members we serve. As Manager, Care Management, you will lead a team of clinical professionals who provide telephonic outreach, assistance and care management to our Special Needs Plan members. We are looking for someone who can communicate with all levels of the organization and motivate team members to meet all established goals.

This is a Remote opportunity, and the schedule is Eastern Standard Time business hours.

This is an opportunity to use your care management knowledge and leadership to support the performance and professional growth of the department. You will work within specific guidelines and procedures; apply advanced technical knowledge to solve moderately complex problems; and determineresources, approaches, schedules, and goals to achieve department objectives. There will also be opportunities to work on projects and initiatives from time to time. The role requires cross-departmental collaboration along with briefings and area meetings; you will maintain frequent contact with other managers across the department.

Key Responsibilities:

  • You will lead a team of approximately 16 - 20 RN professionals who provide telephonic outreach and care management for members.
  • Maintain staffing and processes aligned to support department goals and customer satisfaction.
  • Communicate regularly with team members to motivate, coach, lead,and remove obstacles for them.
  • Lead and empower in a way that supports team member development.
  • Analyze data to interpret trends and identify ways to improve processes or approaches.
  • Monitor department activity to identify opportunities for continuous improvement in both quality and productivity.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


 

$86,300 - $118,700 per year


 

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
