Job SummaryWe are seeking a Licensed Vocational Nurse (LVN) Clinical Coordinator to support home health intake, care coordination, and clinical operations. This role ensures timely admissions, regulatory compliance, and high-quality patient care.
Key Responsibilities- Manage patient intake, referrals, and admissions
- Review documentation for home health eligibility
- Coordinate care with physicians, patients, and families
- Ensure compliance with Medicare, state, and Joint Commission regulations
- Track hospitalizations, holds, and start/resumption of care (SOC/ROC)
- Participate in IDT meetings and QAPI initiatives
- Support field staff with clinical questions and urgent needs
Qualifications- Active LVN license
- 3+ years nursing experience (some home health required)
- Knowledge of home health, intake, or care coordination
- Strong communication and organizational skills
