LPN Unit Manager

Topsfield, Massachusetts

Masconomet Rehabilitation and Healthcare Center

We are seeking an experienced and compassionate LPN Nursing Unit Manager to lead our long-term rehabilitation unit at Masconomet Rehabilitation and Healthcare Center in Topsfield, Massachusetts. As a key member of our healthcare team, you will be responsible for overseeing the day-to-day operation and organization of the unit, ensuring the highest level of quality care for our patients. As an LPN Unit Manager, you will have the opportunity to work with a dedicated team to deliver exceptional patient care and make a meaningful impact in the lives of our patients.

Responsibilities:
• Oversee the day-to-day operation and organization of the long-term rehabilitation unit
• Assist with admissions, participate in family meetings, and discharge planning process
• Attend and participate in daily facility meetings and weekly Clinical Risk and Medicare meetings
• Conduct audits of all new admission charts
• Serve as liaison between family, patient, doctor, and other departments
• Investigate all incident reports and follow-up accordingly
• Participate in managing staff and assignments to ensure quality of care
• May be required to accept on-call hours on weekends and holidays as assigned

Qualifications and Skills:
• Valid LPN license in Massachusetts
• Valid CPR certification
• At least 2 years of nursing supervisory experience
• Excellent computer and electronic medical record skills

Safety & Health Requirements:
• Identifies safety hazards and reports appropriately
• Functions effectively in emergency situations
• Follows facility procedures related to control and cleanliness of equipment and supplies within the unit
• Observes infection control procedures including hand washing and isolation procedures
• Assumes responsibility for the safety of patients under his/her supervision
• Observes all facility safety policies and procedures
• Ensures patient unit is orderly and held to a high standard of cleanliness

Individual Growth Requirements:
• Participates in facility education programs as assigned
• Comes to work in a clean, neat uniform and consistently presents a professional appearance in accordance with department dress code
• Comes to work as scheduled and consistently demonstrates dependability and punctuality
• Accepts assigned duties in a cooperative manner
• Assists others when own work is completed or as directed by Director of Nursing and/or Administrator
• Effectively organizes varying workloads

Documentation Requirements:
• Documents and oversees completion of incidents and/or unusual problems, provision of care, audits and tracks trends in patient care according to established facility procedures
• Oversees thorough and accurate documentation specific to regulatory requirements and overall provision of care
• Takes accountability and assumes responsibility for the overall plan of care for patients on Long Term Rehabilitation unit from admission through discharge; ensuring highest attainable outcomes

General Requirements:
• Follows established performance standards and performs duties according to nursing service policies and procedures
• Assumes accountability for compliance with federal, state and local regulations within the unit assigned and within his/her span of control
• Listens tolerantly and patiently to resident and family complaints, addresses concerns promptly and escalates to Director of Nursing as appropriate
• Follows Resident's Rights policies at all times including confidentiality, privacy and dignity
• Consistently works cooperatively with residents, charge nurses, staff nurses, nursing assistants, physicians, families, consultant personnel, ancillary service providers and administration
• Performs other related duties as directed by Director of Nursing and/or Administrator
