We are currently looking for a Full Time Looapila' Clinical Pharmacy Specialist to join our Outpatient Pharmacy Ambulatory Clinic team located at the Chickasaw Nation Medical Center, working Monday-Friday from 8-5.
Summary:
The purpose of this position is to provide direct patient care within the outpatient pharmacist managed clinics and to deliver the highest quality of patient care and services. Also included within these clinics is the Medication-Assisted Treatment (Suboxone) program. Pharmacist must maintain the highest level of attention to detail, such as drug-drug interactions, drug-disease state interactions, drug-patient interactions (i.e. age, allergies, pregnancy, etc.), and preventing medication errors, all of which have the potential to cause injury or death.
Responsibilities:
* Provides pharmaceutical care services to patients within the Looapila' Clinic
* Checks for duplicate therapy, dosing, refill screens for interactions, and provides complete oral and written instructions on the correct use and storage of all medications to assure complete patient understanding
* Works collaboratively with other clinicians and healthcare providers to provide pharmaceutical care
* Provides drug information to Looapila' medical and nursing staff as needed
* Provides didactic and experiential training in clinical pharmacy for pharmacy students and pharmacy residents
* Functions as a role model for fellow pharmacy staff
* Assists in training pharmacy staff
* Provides pharmaceutical care services to patients within the Medication Assisted Treatment (MAT) Program
* Obtains detailed medical, psychiatric, and substance abuse patient histories
* Assesses patient compliance with therapy through toxicology examination of urinalysis screens, both scheduled and random
* Works collaboratively with MAT team and providers to assess appropriateness of MAT treatment and patient compliance
* Collaborates with MAT team, documents, organizes, and maintains all data collection and information databases relevant to continuing compliance with grant-based funding
* Communicates material to administration and relevant oversight bodies on request
* Assists in performance improvement/quality assurance in various clinical services
* Provides first class customer service to both internal and external customers and responds to inquiries, compliments, and/or complaints with professionalism, courtesy, and efficiency
* Completes all training and development requirements and participates in continuing education opportunities
* Works in a collaborative and supportive manner to support the productivity and safety of the work area, team members and customers of all types
