  • SCOPE OF ROLE:
  • The Licensed Practical Nurse will work as part of a supportive housing team assisting persons served in permanent housing in single site and scatter site.  The LPN is responsible for providing service supports such as medical case management, referrals, health teaching (this might include group education) and counseling.

     

  • ESSENTIAL DUTIES & RESPONSIBILITIES:
    • Conducts physical health assessments.
    • Assesses the health needs of persons served and makes the appropriate referrals to community based medical care.
    • Provides a range of treatment, rehabilitation, and support services to the participants.
    • Visit and provide services to persons served in single site and scattered site
    • Meets regularly with each participant to assess health status
    • Provides health management education, individual and group
    • Review treatment and medication history
    • Communicates with the person served’s primary medical provider to coordinate medical treatment
    • When necessary, refer them to appropriate services as determined as a result of their assessment.
    • Advocate on behalf of person served regarding medical treatment as needed
    • Provide training to program staff on disease, treatment options, and new advances in disease management.
    • Work with consumers in filling prescriptions and coordinating appointments as needed.
    • Maintain CPR/First Aid Certification.
    • Maintain up to date, accurate, complete, and timely consumer chart documentation.
    • Attend staff meetings, case conferences, group supervision, and trainings as needed.
    • Visit residents during hospitalization, attend and contribute to discharge planning meetings to assist with safe discharge.
    • Documents all pertinent information timely and accurately.
    • Any other duties deemed necessary by the Program Director.
    Qualifications

    REQUIRED EDUCATION AND EXPERIENCE

    • Associate Degree in Nursing or a Bachelor of Science Degree in Nursing Must possess an active LPN license in NY state
    • 1+ years working in shelters, behavioral health or with the mental health population, including experience with medication education and management and care plan development.
    Effective written and oral communication skills.
