Qualifications

• One year of clinical LPN experience required.
• Current LPN license in the relevant state(s).
• Current CPR certification necessary.
• Valid driver's license with reliable transportation.
• Liability insurance must be maintained.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to meet patient needs and report changes in condition promptly.
• Teach patients and caregivers self-care techniques effectively.
• Check and record vital signs during patient visits.
• Review medications and perform medication reconciliations at each visit.
• Execute specialized procedures as ordered by the physician, like foley catheter changes.
• Prepare necessary equipment and materials for treatments using aseptic technique.
• Complete accurate documentation in a timely manner.
