What You'll Do- Carry out individualized treatment plans developed by the supervising Physical Therapist.
- Provide therapeutic exercises, gait training, transfer training, balance activities, and functional mobility interventions.
- Reinforce home exercise programs and educate patients and caregivers.
- Monitor patient progress and communicate updates with the supervising PT.
- Collaborate with physicians, nurses, and the interdisciplinary care team.
- Complete accurate and timely clinical documentation.
What We're Looking For- Active Minnesota Licensed Physical Therapist Assistant (PTA) license.
- Passion for helping patients regain independence.
- Strong communication, organization, and patient education skills.
- Home health experience is preferred but not required.
- Valid driver's license and reliable transportation.
