Full Job Description
Schedule: Monday-Friday 8am-5pm
Pay Range: $90,000-95,000 per year, based on experience
Location: This position will be based out of our Partners in Health office in Westminster and will travel between all 4 of our clinics. Partners in Health, Westminster, Arvada, & Lakewood
Essential Duties:
Provide exceptional behavioral health interventions, within an integrated care setting, utilizing therapeutic intervention models (brief solution focused, CBT, MI, ACT etc.)
Experience and willingness to work with a diverse population with a wide range of mental health challenges, including substance abuse.
Work effectively and collaboratively with the primary care medical providers to deliver whole person care
Coordinate care with other community providers and provide linkage and referrals to other services as needed.
Set and monitor achievable clinical goals and outcomes
Participation in provider team meetings.
Ability to provide both in person and telehealth treatment.
Willingness to explore other clinical interventions that may benefit the patients of the practice (i.e., clinical groups or other modes of providing care as deemed appropriate and beneficial)
