Full Job Description
Schedule: Monday-Friday 8am-5pm

Pay Range: $90,000-95,000 per year, based on experience

Location: This position will be based out of our Partners in Health office in Westminster and will travel between all 4 of our clinics. Partners in Health, Westminster, Arvada, & Lakewood

Essential Duties:
  • Provide exceptional behavioral health interventions, within an integrated care setting, utilizing therapeutic intervention models (brief solution focused, CBT, MI, ACT etc.)
  • Experience and willingness to work with a diverse population with a wide range of mental health challenges, including substance abuse.
  • Work effectively and collaboratively with the primary care medical providers to deliver whole person care
  • Coordinate care with other community providers and provide linkage and referrals to other services as needed.
  • Set and monitor achievable clinical goals and outcomes
  • Participation in provider team meetings.
  • Ability to provide both in person and telehealth treatment.
  • Willingness to explore other clinical interventions that may benefit the patients of the practice (i.e., clinical groups or other modes of providing care as deemed appropriate and beneficial)
  • Timely documentation
  • Other duties as assigned


  • Master's Degree (from accredited university/college) in Social Work
  • License required: LCSW

  • Experience coordinating care within the community to wrap around services that may not be available within the primary care setting.
  • Ability to assess and appropriately manage crisis situations that require risk evaluation and management.
