Full Job Description
This position would cover Skagit County (Mount Vernon, Burlington, Sedro-Woolley, Anacortes, La Conner, Big Lake, Bow, Alger) for home health skilled nursing services.

This position works Monday through Friday day shift, with self-directed scheduling dependent on patient needs and territory. If you enjoy a high level of autonomy, travel and meeting patients where they are, apply today!

Duties and Responsibilities
  • Provides direct patient care according to the plan of care, such as but not limited to wound care, catheterization, disease process education, medication management and education.
  • Works with the Registered Nurse Case Manager to develop the plan of care in partnership with the patient's primary care provider.
  • Completes documentation timely, evaluates patient's needs and records pertinent information into the chart.
  • May be required to work outside of the primary assigned area due to staffing and census fluctuations.
  • Participates in on-call duties and takes assigned on-call shifts throughout the month, usually 3-4 weekdays per month and 1-2 weekend days per month.


Knowledge, Skills and Experience
  • Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home health preferred.
  • Currently licensed as a Practical Nurse (LPN) in Washington State.
  • Possesses and maintains CPR certification.
  • Must be licensed and able to practice in Washington State.
  • The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job. Must have and maintain a valid driver's license, maintain automobile insurance coverage and have access to an automobile.


Preferred Qualifications:
  • Clinicians with experience in Home Health or post-acute care.
  • Familiarity with Homecare Homebase EMR.


Schedule, Pay and Benefits
  • Full-time (32+ hours a week) Monday through Friday.
  • Hourly $37 - 42 (Dependent on Experience) + Mileage Reimbursement.
  • Medical/Dental/Vision available the 1st of the month after hire.
  • Paid company holidays, 2+ weeks PTO and 401k match after 90 days.
