Job Summary: The Hospitalist Care Facilitator assesses, plans, implements, and coordinates the discharge plan as directed by Hospitalist Program to maximize Hospitalist's efficiency. This role maximizes the efficient utilization of resources.

Minimum Education: Has completed the basic professional curricula of a school of nursing as approved and verified by a state board of nursing, and holds or is entitled to hold a diploma or degree therefrom.

Licensure, Registration and/or Certification: Valid multi-state or State of Oklahoma Registered Nurse License.

Work Experience: Minimum 5 years of related experience.

Knowledge, Skills and Abilities: Working knowledge of general disease state management processes, bio-psychosocial model, and clinical pathways. Effective interpersonal, written, and verbal communication skills. Ability to organize and prioritize work in an effective and efficient manner. Ability to utilize PC & software skills.

Essential Functions and Responsibilities: Assists with improving patient flow by expediting discharge orders and fielding questions/issues via telephone, secure chat and inbox messaging that needs to be discussed with physicians. Consults and collaborates with treatment team (Hospitalists, Physicians, Advanced Practice Providers, Registered Nurse Case Managers, Social Workers, Educators, and/or other healthcare personnel) as appropriate to effectively coordinate patient discharge as ordered by the attending physician. Participates in providing outstanding customer service and accepts responsibility for maintaining relationships that are equally respectful to all. Collaborates with the Hospitalists and the hospital interdisciplinary care team to coordinate and manage the process of patient care. Oversees the assimilation of information related to assessment, planning, implementation, and evaluation of patient care needs to maximize the efficient utilization of resources.

Decision Making: Independent judgement in planning sequence of operations and making minor decisions in a complex technical or professional field.

Working Relationship: Coordinates activities of others (does not supervise). Leads others in same work performed (does not supervise). Works directly with patients and/or customers. Works with internal and/or external customers via telephone or face to face interaction. Works with other healthcare professionals and staff.
