Registered Nurse (RN) - Hospice Case Manager - Full Time

$85,000-$100,000 per year + bonuses
Monday-Friday | 8:00 AM-5:00 PM | Evenings/weekends sparingly
Hospice RN Opportunity - Compassionate Care, Strong Support, Real Balance

This is a full-time hospice nursing position providing in-home end-of-life care with a supportive interdisciplinary team, reasonable expectations, and leadership that understands hospice nursing.
Job Summary

The Hospice Registered Nurse Case Manager (RNCM) is responsible for coordinating and delivering skilled nursing care to hospice patients in their homes. You will collaborate with physicians and the interdisciplinary team (IDT) to ensure comfort, dignity, and high-quality end-of-life care.

This role is ideal for an RN with hospice, home health, or case management experience who values autonomy, meaningful patient relationships, and work-life balance.
Key Responsibilities
  • Perform routine visits, symptom management visits, and hospice admissions on occasion (when admissions RN is unavailable)
  • Develop, manage, and update individualized hospice plans of care
  • Collaborate with hospice physicians and the interdisciplinary team (IDT, inclusive of home health aides, social workers, and chaplains
  • Provide symptom management and end-of-life nursing care
  • Educate patients and families on disease progression, medication management and hospice services
  • Complete accurate and timely documentation per organizational policy
  • Participate in care conferences, building introductions and team meetings
Qualifications

  • Hospice or home health nursing experience preferred
  • Strong clinical judgment and communication skills
  • Ability to work independently in the community setting
  • Valid driver's license, reliable vehicle, and auto insurance
