Full Job Description
Key Responsibilities

The Hospice Nurse Preceptor is a trained professional nurse who administers skilled nursing care to patients and families who are terminally ill and meet hospice admission criteria in the home or facility setting. May be required to perform admission, routine, respite, or continuous care visits and assess patients within regulatory requirements for hospice. Responsible for overall coordination of the care delivered to assigned patient caseload. This is accomplished by making regular visits to patients wherever they reside, communicating regularly with the Interdisciplinary Team, Medical Director, identifying patient, family, caregiver needs and providing for those needs in accordance with the Attending Physician's orders and the IDG plan of care. This position is also responsible for the training of clinicians, including during the new hire orientation process and remedial training when required. May also perform facility education when requested.

In this role you will be responsible for:
  • Makes follow up visits according to patient, family & facility needs
  • Assumes primary responsibility for caseload, including assessing, planning, coordinating, implementing and evaluating the plan of treatment
  • Possesses basic assessment skills specific to the provision of Hospice care, including but not limited to identification of home safety risks, appropriateness of home environment to the delivery of care, availability and capacity of support systems and community resources
  • Initiate, supervise and document observance of the Home Health Aide and LPN delivery of care per organization policy
  • Assumes responsibility for the clinical training of students as well as remedial training of tenured staff when warranted
  • Maintains regular communication with Education Departments and student's supervisor
  • Assures students meet Optimal Care expectations for productivity, time to synch, timely documentation completion, length of visits and other metrics as defined by role.

  • Current State License as a Registered Nurse
  • Minimum one (2) year experience as a nurse in a hospice or home health environment
  • Nursing skills as defined by accepted nursing standards
  • Demonstrates skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
  • Possess excellent observation, analytical, verbal, and written communication skills
  • Demonstrates the ability to assess and respond to the needs of the organization
  • Strong leadership skills to be able to train clinicians
  • Must be able to communicate professionally with all levels of the organization
  • Reliable transportation and valid automobile insurance coverage

Pay Range

$75,000-$88,000 USD
