Hospice Nurse Practitioner (NP) - Palliative & End-of-Life Care

Field-Based | Patient & Family-Centered Care

As a Hospice Nurse Practitioner (NP), you'll bring advanced clinical expertise and compassionate care directly to patients and families during one of life's most meaningful transitions. Working in collaboration with our physicians and interdisciplinary hospice team, you'll assess patients, manage symptoms, complete face-to-face evaluations, and help ensure each patient receives care aligned with their needs and goals.

What You'll Do

As a Hospice Nurse Practitioner, you'll provide advanced clinical support to hospice patients while working closely with the Hospice Medical Director, attending physicians, and interdisciplinary team.

You'll:
  • Conduct comprehensive patient assessments, histories, and physical examinations.
  • Provide medical care and symptom management for patients with terminal illnesses.
  • Develop individualized plans of care and recommend appropriate medications, treatments, consultations, and referrals.
  • Complete required hospice face-to-face (F2F) assessments and evaluate continued hospice eligibility.
  • Provide clinical findings and recommendations to the Hospice Medical Director for certification and recertification decisions.
  • Collaborate with the interdisciplinary team to evaluate changing patient needs and adjust the plan of care.
  • Help ensure patients receive appropriate interventions for pain, symptoms, comfort, and quality of life.
  • Educate and support patients, families, and caregivers as they navigate end-of-life care.
  • Participate in interdisciplinary group meetings and contribute clinical expertise to care planning.
  • Maintain timely, accurate clinical documentation and ensure compliance with hospice regulatory requirements.
  • Serve as a clinical resource and provide education and consultation to members of the hospice team.
  • Build collaborative relationships with staff and community referral partners.
What We're Looking For

We're looking for an NP who combines strong clinical judgment with genuine compassion - someone comfortable navigating complex medical needs while helping patients and families understand their options and goals of care.

Qualifications:
  • Graduate of an accredited School of Nursing and accredited Nurse Practitioner program.
  • Master's degree.
  • Current applicable nursing/APRN licensure.
  • Current CPR certification.
  • Minimum of 1 year of Nurse Practitioner experience.
  • Minimum of 1 year of Hospice experience.
  • Advanced practice certification from an approved certifying body as required by the state.
  • Strong understanding of hospice care, end-of-life symptom management, and patient/family education.
  • Knowledge of applicable Medicare/Medicaid and state/federal hospice regulations.
  • Valid driver's license with an insured, reliable vehicle.

Preferred:
  • Hospice and Palliative Nursing certification or other applicable specialty certification.
  • Experience completing hospice face-to-face assessments and supporting certification/recertification.
  • Strong knowledge of Medicare Conditions of Participation (CoPs).
