As a Nurse Practitioner, you will play a vital role in ensuring patients and families receive exceptional, patient-centered hospice care. You'll collaborate with our interdisciplinary team and serve as a trusted resource for patients, families, and the community.

Duties of the Nurse Practitioner:
  • Collaborate with the Interdisciplinary Team to evaluate patients and develop palliative treatment plans
  • Assist in the planning, implementation, operation, and evaluation of hospice care for terminally ill patients and their families
  • Participate in IDT meetings and contribute to the plan of care
  • Partner with attending and consulting physicians to ensure coordinated care
  • Provide timely and accurate Face-to-Face visits in compliance with regulations
  • Offer guidance to staff and referring physicians on medical interventions consistent with hospice philosophy
  • Assist in preparing educational materials for staff, patients, families, and community groups
  • Participate in infection control, QAPI, and on-call rotations as needed
  • Make patient care decisions with professionalism, compassion, and clinical expertise
  • Support staff orientation, continuing education, and community outreach initiatives


Job Requirements
  • Licensed Nurse Practitioner in the state of Indiana
  • Knowledge of primary medical care principles, with a working knowledge of oncology preferred
  • Strong background in palliative care, with emphasis on pharmacologic symptom control
  • Understanding and acceptance of hospice philosophy of care
  • Knowledge of growth and development principles across the life span
  • Ability to assess and interpret patient data for age-specific needs
  • Understanding of the grief process for patients and families
  • Excellent communication, critical thinking, and leadership skills
  • Reliable transportation and auto insurance
