Job Profile Summary

This role focuses on providing professional and nonprofessional nursing care services in accordance with physician orders. In addition, this role focuses on performing the following Home Care Nurse duties: Assesses, plans, implements, and evaluates the care needs of a designated group of patients or provides nursing care and identified clinical support tasks within the scope of practice for achievement of the patient's plan of care as directed. Provides professional nursing care to patients. Requires a nursing license. A professional individual contributor role that may direct the work of other lower level professionals or manage processes and programs. The majority of time is spent overseeing the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typically acquired through advanced education. A role that supervises all levels of employees. Responsibilities that typically include: Setting goals and objectives for team members for achievement of operational results, problems faced may be difficult but typically are not complex, and ensures policies, practices and procedures are understood and followed by direct reports, customers and stakeholders.

Job Overview

The Senior Hospice Clinical Manager ensures delivery of quality care through supervision and leadership of an interdisciplinary team; Utilizes resources in a cost effective and efficient manner. In addition to the full scope of the Clinical Manager role, this role assists the Clinical Director in ensuring optimal operations and care delivery are in place. This role also works collaboratively with peer managers in strengthening all teams.

Job Description

Minimum Qualifications:

1. Bachelor of Science in Nursing (BSN).

2. Registered Nurse (RN) license.

3. Two (2) years of hospice OR (1) year of management experience.

Location: Lowell, MA

Hours: Full-Time, 40hrs/week

Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.

1. Works with Medical Director to deliver optimal hospice care.

2. Teaches hospice philosophy including compassion, clinical expertise, and promotes service excellence to employees.

3. Ensures contribution from all members of the IDT in the delivery of hospice care for each patient.

4. Utilizes team building and problem-solving skills to promote optimal team performance, effectiveness, efficiency.

5. Addresses and resolves complaints promptly.

6. Completes assigned projects and reports per deadline.

7. Maintains continuity of care by IDT when staff are on vacation, sick, etc.

8. Leads IDT meetings.

9. Ensure status changes are left promptly and correctly.

10. Conducts random chart audits. Follows through on any areas that appear to be deficient or incorrect according to the Hospice regulations and policies.

11. Approves only invoices related to the delivery of hospice care.

12. Develops effective relationships with referral sources.

13. Works with UR to ensure availability of documentation and information needed for payment.

14. Hires staff with appropriate skills and experience for available positions.

15. Acts as point contact leader within Hospice when Director of Hospice and Palliative Care is not present or readily available.

16. Keeps Director of Hospice and Palliative Care apprised of activities and issues.

17. Serves as Clinical Administrator on call.

18. Works with Director to achieve goals and vision of organization.

19. Supports grant and fundraising activities.

20. Assists the Director of Hospice and Palliative Care with capacity management daily

21. Attends appropriate conferences, CEU programs and seminars to keep up to date with professional practice and current trends in hospice care and management.

22. Works closely with scheduler and clinical coordinator overseeing scheduling of overflow visits with other managers and alerts Director of Hospice and Palliative Care of any capacity issues (caseloads/visit/admissions) that cannot be resolved at the manager level.

23. Reports Compliance concerns to the CEO or Chief Compliance Officer when applicable.

24. Ensures compliance within guidelines set forth by regulatory agencies (DPH, ERISA etc.) and demonstrates compliance with Tufts Medicine Care at Home policies and procedures.

25. Ensures delivery of quality care in a cost-effective manner by providing supervision and leadership to clinical team.

26. Maintains productivity levels on team.

27. Holds staff accountable to current clinical standards of practice and policies and procedures.

28. Coaches and develops staff to achieve maximum potential. Monitors staff performance for accountability.

29. Addresses employee performance issues in a timely manner.

30. Maintains documentation on employee performance.

31. Prepares and delivers balanced Performance Evaluations in a timely manner.

32. Works in close collaboration with other clinical managers in census management, ensuring case load review is performed consistently, SIA management and LOS strategies are in place.

33. Works in collaboration with HR on personnel placement.

34. Attends recruitment meetings with Director of Hospice and Palliative Care.

35. Oversees pre-weekend planning as needed assuring that all priority patients are accounted for on the schedule.

36. Works closely with Director of Hospice and Palliative Care in oversight of work queue deficiencies are cleared across all teams and plays an integral role of onboarding new hospice staff/clinical managers.

Physical Requirements:

1. Frequently required to move about inside the office to access file cabinets, office machinery, etc.

2. Remain in a stationary position 90% of the time.

3. Occasionally lift and/or move up to 10-25 lbs.

4. Constantly operates a computer and other standard office equipment such as copy machine, phones, photocopier, fax machines and computer printers.

5. Frequently communicates internally and externally; must be able to exchange accurate information via phone, email and/or in person.

6. Specific vision abilities apply including close vision, distance vision and ability to adjust focus.

7. Clerical office setting.

8. Noise level in the work environment is usually moderate.
