Licensed Practical Nurse (LPN), you'll play a critical role in providing compassionate, patient-centered care in a home health setting. Working closely with Registered Nurses and other healthcare professionals, you'll support patients in their journey toward recovery and quality of life.
What You'll Do:- Provide Exceptional Care: Implement care plans under the direction of Registered Nurses, ensuring the highest standards of patient care.
- Empower Patients: Assist clients with preventative and rehabilitative nursing procedures to improve their health and well-being.
- Collaborate with a Caring Team: Build strong relationships with patients, families, and healthcare providers to deliver coordinated care.
- Monitor Care Quality: Ensure that services are delivered with attention to quality and appropriateness.
- Follow Safety Protocols: Maintain a safe, clean environment by adhering to infection control and organizational policies.
Who We're Looking For:We want someone who is passionate about providing exceptional care and making a positive impact in the lives of patients. The ideal candidate will have:
- Education & Licensing: Graduation from an NLN-accredited LPN program with active New Jersey LPN licensure.
- Experience: 2+ years of med/surg or acute care nursing experience. Home care or hospice experience is a plus!
- Skills: Strong verbal and written communication skills, excellent computer skills, and exceptional organizational abilities.
- A Team Player: Ability to collaborate and foster positive, professional relationships.
- Driver's License & Transportation: You'll need a valid driver's license and reliable transportation for local travel.
