Description

SUMMARY: Responsible for placing, monitoring and assessing intravenous systems, medications and/or products. Utilizing physicians' orders and professional skills, the Home Infusion Therapy Services RN develops and implements a plan that meets each patient's specific needs and is in compliance with accrediting organization standards, federal and state regulations, reimbursement guidelines, as well as Agency policies and procedures

Scheduling: Monday - Friday 8 am to 430 pm

  • Education: Completion of an accredited Registered Professional Nursing Program, Bachelor's degree preferred
  • Experience:
    • Strong IV and central line skills
    • Previous experience administering IV medications and infusion therapies
    • Home Care experience or outpatient infusion therapy preferred
    • Adult and Pediatric experience preferred
  • Licensure, Certification, and/or Credential Requirements
    • Current New York State RN
    • Current BCLS Certification
    • Valid NY State Driver's License and reliable transportation


Key Responsibilities
  • Perform comprehensive initial and ongoing patient assessments, develop and update individualized plans of care, and evaluate continued care needs.
  • Provide infusion therapy and skilled nursing care to patients of all ages, from neonates through geriatrics.
  • Administer and monitor specialty medications, antineoplastic agents, parenteral nutrition, and pain management therapies while following medication safety standards.
  • Monitor infusion sites and systems, perform venous punctures and phlebotomy, and maintain aseptic technique and infection prevention practices.
  • Assess patients for treatment-related adverse reactions, complications, and pain; implement appropriate interventions and provide education to patients and caregivers.
  • Collaborate with physicians and the multidisciplinary care team, obtain necessary orders, communicate significant lab results, and coordinate ongoing patient care.
  • Educate patients and caregivers on treatment, medications, and ongoing care; ensure appropriate supplies, equipment, documentation, and follow-up are maintained.
  • Support a culture of patient safety, quality, regulatory compliance, and continuous improvement, including participation in staff meetings, education, and QAPI activities.
  • Maintain accurate and timely clinical documentation while adhering to organizational policies, confidentiality standards, and professional expectations.
  • Other duties as assigned.
