Description
Reliable Home Care Providers, Inc. is hiring a bilingual RN (RUSSIAN/ENGLISH)!Join our growing team of healthcare professionals as a home health nurse!

Responsibilities:- Completing the nursing process in the delivery of patient care, which includes assessment, planning, implementation, and evaluation of patient outcomes.
- Total oversight of patient from admission to discharge.
- Developing a plan of care based on findings.
- Serving as patient's advocate and maintaining ongoing communication with physicians, referral sources, caregivers, and facility management to coordinate care.
- Timely completion of all skilled nursing documentation.
- Providing direct supervision of nursing clinical care team members (LPN/LVNs & HHAs)
- Multidisciplinary team approach (coordinating with all disciplines of patient care, including PT, OT, ST, MSW, HHA's, and office staff)
Requirements
- Current Illinois State RN.
- CPR Certification
- Previous experience in nursing or other health field experience
- Familiarity with medical software and equipment
- Ability to build rapport with patients
- Strong problem-solving and critical-thinking skills
- Ability to thrive in a fast-paced environment
- WellSky EMR knowledge is preferred but not required.
Compensation and benefits:- Salary Range: $65-$85/per visit, commensurate with experience
