Home Health RN for Skilled Visits to service San Antonio and surrounding areas.** Travel Time and Mileage Paid ****** Medicare SOCs / Recerts : $$115.00**** Non-Medicare SOC's / Recerts : $90.00**** Treatments/DC : $50.00**5K Retention after a year with the company** (Must be Full-Time)**LOOKING SPECIFICALLY FOR TRUCARE** and REGULAR VISITS.What You'll Do:- Conduct comprehensive assessments of patients' medical conditions and needs.
- Develop and implement individualized care plans in collaboration with the team.
- Administer medications, treatments and procedures as prescribed by physicians.
- Monitor patients' responses to treatments and make necessary adjustments to care plans.
- Maintain accurate and timely documentation of all patient interactions, treatments, and progress.
- Communicate effectively with patients, families, and other healthcare providers to ensure coordinated and optimal patient care.
Qualifications:- Current and valid Registered Nurse (RN) license in the state of Texas
- CPR & BLS certification.
- Strong clinical skills and knowledge of home health care practices.
- Ability to work independently and manage time efficiently.
