Essential Duties and Responsibilities:- Develop, implement, and adjust a conditioning/rehabilitation plan of care in partnership with patients, representatives, caregivers, and physicians
- Test and screen patients' physical strength to assist physicians in evaluating their level of function
- Educate and instruct patients, family members, or representatives about exercise programs and therapeutic procedures
- Accurately document observations, interventions, and evaluations pertains to patient care management and services provided
- Provide direct patient care as prescribed, including evaluation and treatment planning
- Review physicians' referrals and patients' medical records to determine diagnosis and physical therapy treatment required
Minimum Qualifications (Knowledge, Skills, and Abilities):- Graduation from an approved Physical Therapy curriculum
- Current and valid Connecticut Physical Therapist license
- Recent work experience in home health (preferred)
- Driver's License, Car Insurance, and Car Registration
- Strong clinical assessment, organizational, and communication skills
- Ability to handle multiple assignments simultaneously
- Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing, and documentation
- Bilingual (Spanish) preferred
- Strong interpersonal skills and good judgment
- Ability to work well under pressure and stay calm and focused in emergent situations
- Be highly self-motivated and results-driven
- A desire for further education to keep up with the ever-changing medical advances

Salary:- $90,000 - $120,000 ANNUALLY
