Home Health Physical Therapist PT

Job Summary:

As a Home Health Physical Therapist, you will work with patients in their homes to improve mobility, relieve pain, increase strength, and promote overall well-being. Essential Duties and Responsibilities:
  • Develop, implement, and adjust a conditioning/rehabilitation plan of care in partnership with patients, representatives, caregivers, and physicians
  • Test and screen patients' physical strength to assist physicians in evaluating their level of function
  • Educate and instruct patients, family members, or representatives about exercise programs and therapeutic procedures
  • Accurately document observations, interventions, and evaluations pertains to patient care management and services provided
  • Provide direct patient care as prescribed, including evaluation and treatment planning
  • Review physicians' referrals and patients' medical records to determine diagnosis and physical therapy treatment required

Minimum Qualifications (Knowledge, Skills, and Abilities):
  • Graduation from an approved Physical Therapy curriculum
  • Current and valid Connecticut Physical Therapist license
  • Recent work experience in home health (preferred)
  • Driver's License, Car Insurance, and Car Registration
  • Strong clinical assessment, organizational, and communication skills
  • Ability to handle multiple assignments simultaneously
  • Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing, and documentation
  • Bilingual (Spanish) preferred
  • Strong interpersonal skills and good judgment
  • Ability to work well under pressure and stay calm and focused in emergent situations
  • Be highly self-motivated and results-driven
  • A desire for further education to keep up with the ever-changing medical advances

  • $90,000 - $120,000 ANNUALLY
