Duties & Responsibilities

Purpose of Position:

Under the direction of supervision of Physical Therapy, provides home visits for diagnostic and therapeutic physical therapy services to home health patients. Examines and evaluates the patient's condition and plans and administers treatments to promote optimal health. Seeks to relieve pain, improve the body's movement and function, maintain cardiopulmonary function, restore, maintain and promote optimal physical function and limit disabilities resulting from injury or disease. Acts in compliance with applicable local, state, and federal rules and regulations and physical therapy standards of care.

Essential Duties and Responsibilities:
1. Provides physical therapy services, in accordance with the physician's plan of care.
2. Assists the physician in assessing the patient's functional level by applying appropriate tests and measurements.
3. Evaluates the patient and establishes/upgrades the plan of treatment by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests.
4. Treats the patient through the use of therapeutic activities to relieve pain, develop and restore function, attain and maintain maximum performance and utilize adaptive equipment within the functional limitations of the patient and in accordance with the plan of care.
5. Regularly observes signs and symptoms of the patient's response to treatment and rehabilitation needs. Documents and reports findings to the physician and/or other appropriate health professional.
6. Notifies the coordinator of care and/or the physician of the patient's development and the need for revisions to the patient's plan of care. Ensures that the plan of care addresses all problems identified during the evaluation or demonstrates rationale for not doing so.
7. Develops, prepares and maintains individualized patient care documentation with accuracy, timeliness and in accordance with the System's policies.
8. Assists with the coordination of the plan of care and maintains continuity of patient care through interdisciplinary coordination with other health professionals by attending patient care conferences. Makes referrals to other disciplines, as indicated by the needs of the patient or document rationale for not doing so.
9. Evaluates the home environment and makes appropriate recommendations with regard to safety issues.
10. Orders and/or fabricates adaptive/assistive devices and equipment and trains the patient in the safe and effective use of the equipment.
11. Instructs, trains and advises the patient, family members and other home health personnel in the phases of physical therapy in which they may assist the patient, and in the safe and effective use of equipment, assistive devices, and the completion of the home exercise program.
12. Demonstrates and teaches alternate techniques to perform activities of daily living, transfers, and mobility and positioning.
13. Teaches, supervises and counsels the patient and designated caregiver regarding home care procedures, as appropriate to the patient's condition.

Minimum Qualifications

New York State license and current registration to practice as a Physical Therapist; and

One year of post license experience as an Physical Therapist.

Holds, or obtains, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

Department Preferences
1. Provides physical therapy services in accordance with the physician's plan of care.

5. Observes, documents and reports the patient's responses to treatment and any change in the patient's condition to the physician, and/or the coordinator of care.
6. Evaluates the home environment and makes appropriate recommendations with regard to safety issues.
7. Orders and/or fabricates adaptive/assistive devices and equipment and trains the patient in the safe and effective use of the equipment.

10. Regularly reviews the rehabilitation needs with patients; initiates/notifies the coordinator of care and/or the physician regarding the need for revisions of the patient's plan of care. Ensures that the plan of care addresses all problems identified during the evaluation or demonstrates rationale for not doing so.
11. Makes referrals to other disciplines as indicated by the needs of the patients or documents rationale for not doing so. Observes signs and symptoms and reports to the physician and/or other appropriate health professionals as often as needed or as directed by the changes in the patient's condition.
12. Assists with the coordination of the plan of care and maintains continuity of patient care through interdisciplinary coordination with the other health professional by attending patient care conferences.
13. Teaches, supervises and counsels the patient and designated caregiver regarding home care procedures as appropriate to the patient's condition.
14. Develops, prepares and maintains individualized patient care documentation with accuracy, timeliness and in accordance with the Corporation's policies.
15. Performs SOC Assessments for Joint Replacement cases, following specified Joint Replacement Protocol.
16. Participates in interdisciplinary case conferences and case load reviews to ensure optimal clinical outcomes for assigned patients.
17. Escalates patient care concerns in a timely manner to Assistant Director of Nursing or Assistant Director of Rehab Services to prevent patient decline and potential rehospitalization.
18. Collaborates with the patient and/or patient identified representative in developing appropriate treatment goals in alignment with the physician ordered plan of care.
19. Participates in the agency's Quality Assurance Performance Improvement/(QAPI) program Provides Field

Evaluations to therapists as per regulatory requirements.
Following are the working conditions:
1. Daily home visits require walking, driving or public transportation between visits.
2. May be required to climb stairs to make home visits and communicate with the office regularly before and at the end of the day and possibly between patient visits.
3. Travel between/to other boroughs may be required as needed.
4. Will be required to carry an agency issued laptop, clinical bag and cell phone.

EQUIPMENT/MACHINES OPERATED:
Smart phone - Laptop Computer - Desktop Computer - Multifunction Copier
DME appropriate for patient care, i.e., wheelchairs, canes, walkers, transfer devices, TheraBands, gait belts, etc.

COMPUTER PROGRAMS/SOFTWARE OPERATED
EPIC H2O Hyperspace - preferred but not mandatory. EPIC H2O Remote Client - preferred but not mandatory.
Microsoft Office (Outlook, Word, Excel, PowerPoint, Visio, OneNote)

KNOWLEDGEABLE IN:
All aspects of a Qualified Physical Therapist skill set. Knowledge of computers. Some knowledge of Home Care regulations preferable but not mandatory.
