About the Role:We are seeking a highly skilled Physical Therapist to join our team as a PT for Home Health. Minimum Qualifications:- Current state license as a Physical Therapist
- Minimum of 1 year of experience in home health
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and critical thinking skills
Responsibilities:- Conducting patient evaluations and developing individualized treatment plans
- Providing hands-on therapy to patients in their homes
- Educating patients and their families on proper techniques and exercises
- Collaborating with physicians, nurses, and other healthcare professionals to ensure the best possible patient outcomes
- Maintaining accurate and up-to-date patient records
