The RN Home Health provides comprehensive, patient-centered nursing care in the home setting, coordinating and delivering individualized care plans across the continuum. Utilizes clinical expertise, critical thinking, and collaboration to promote optimal patient outcomes and support safe, effective care transitions.
Home Health Nurse. Mileage Reimbursement. Company equipment. Franklin County Region.
  • Assesses, plans, implements, and evaluates patient care to achieve optimal outcomes across all age groups as appropriate to the service area.
  • Performs comprehensive admission and ongoing assessments and develops individualized plans of care.
  • Collaborates with patients, families, and interdisciplinary team members to implement and evaluate care plans, revising as patient needs change.
  • Administers medications and treatments accurately and documents care in the electronic medical record.
  • Identifies and addresses patient and caregiver educational needs and incorporates teaching into the plan of care.
  • Applies knowledge of growth and development to deliver age-appropriate care.
  • Advocates for patients and supports their rights, providing psychological, social, and cultural support.
  • Utilizes clinical judgment and problem-solving skills in dynamic care environments.
  • Manages resources, prioritizes tasks, and adapts to meet patient care needs.
  • Supports performance improvement initiatives and contributes to quality outcomes.
  • Participates in orientation and training of new employees and supports ongoing staff development.
  • Communicates effectively with healthcare team members to coordinate care and ensure continuity.
  • Role model for clinical practice.

What We Require:

Education: Graduate of an accredited school of professional nursing required.

Experience: Minimum of 1 year of nursing experience required.

Licensure, Certification, and/or Registration: Current licensure as a Registered Nurse (RN) in the Commonwealth of Virginia required. Valid Virginia driver's license required for home health roles.

Life Support: AHA BLS-HCP certification required.

Other Minimum Qualifications: Ability to meet orientation and competency requirements. Strong communication, critical thinking, and organizational skills. Proficiency with electronic medical records and computer systems. Ability to work independently and manage patient care in a home setting.

Compensation Range:
$76,689.60 - $115,044.80
Employee pay is based on a variety of factors, including qualifications, credentials, education, and prior work experience. The compensation range does not include benefits or pension plan.
