Current Washington State LPN license or eligibility before starting
Prior home health or hospice experience preferred
Ability to work independently and demonstrate strong clinical judgment
Current BLS certification from AHA
Valid driver's license and reliable, insured vehicle
Responsibilities
Provide one-on-one skilled nursing care in patients’ homes, including wound and catheter care
Conduct skilled observation, assessment, and evaluation of patient conditions
Educate and support patients, families, and caregivers on health management
Collaborate effectively with healthcare providers and interdisciplinary teams
Manage a caseload of 5–6 patients per day

What You’ll Be Doing
Provide one-on-one skilled nursing care in patients’ homes (e.g., wound care, catheter care, medication administration, symptom management)
Conduct skilled observation, assessment, and evaluation
Educate and support patients, families, and caregivers
Collaborate with healthcare providers and the interdisciplinary team
See an average of 5–6 patients per day
What We’re Looking For
Current Washington State LPN license (or eligible before your first day)
