Home Health Admissions RN | Three-Day Full-Time ScheduleSaturday–Monday | Newburyport to Haverhill
$88,000–$105,000, based on experience | Up to $5,000 sign-on bonus

The first home health visit sets the tone for everything that follows. At Elara Caring, we’re looking for an experienced RN who can walk into a patient’s home, see the full picture, and build a thoughtful plan for what comes next.
As an Admissions RN, you’ll focus on start-of-care visits across Greater Boston and the North Shore. You’ll have the independence to use your clinical judgment, with a team behind you to help patients move smoothly into ongoing care.
What your work will look like- Complete start-of-care visits and comprehensive assessments
- Evaluate clinical needs, medications, home safety, and caregiver support
- Create individualized plans of care with physicians and the clinical team
- Educate patients and families so they understand the plan and feel prepared
- Document admissions accurately and communicate key findings to the care team
What you’ll bring- An active, unrestricted Massachusetts RN license
- At least one year of nursing experience
- Home health experience is required; admissions or start-of-care experience is preferred
- Strong assessment, documentation, and independent decision-making skills
- A valid driver’s license, reliable transportation, and current auto insurance
Experience with HomeCare HomeBase (HCHB) is a plus. An associate degree in nursing is required; a BSN is preferred.
